Continuing Care

Patients leaving Pavillon after treatment receive a continuing care plan that may include transition to other levels of care and post treatment needs. The process results in the creation of a workable plan that reflects the patients strengths, needs and preferences. After treatment, Pavillon staff follow up with each patient via telephone calls at various intervals. This process provides the patient as well as staff with feedback that is supportive of their recovery needs at various stages. This may also lead to the need for referral to additional treatment services as assessed in the follow-up calls. Continuing care services may include weekly outpatient group sessions led by a licensed or certified counselor or master's level clinician.
