Continuing Care

Patients leaving Pavillon after treatment receive a continuing care plan that may include transition to other levels of care and post treatment needs.  The process results in the creation of a workable plan that reflects the patients strengths, needs and preferences. 

After treatment, Pavillon staff follow up with each patient via telephone calls at various intervals.  This process provides the patient as well as staff with feedback that is supportive of their recovery needs at various stages.  This may also lead to the need for referral to additional treatment services as assessed in the follow-up calls.

Continuing care services may include weekly outpatient group sessions led by a licensed or certified counselor or master's level clinician. Sessions include a general curriculum covering basic recovery needs.

Important continuing care issues covered in early recovery include:

  • Getting active in 12-Step meetings
  • Completing the 10th Step on a regular basis
  • Sponsorship
  • Accountability groups
  • Building and maintaining relationships in recovery
  • Stress and how it triggers relapse
  • Wellness and balance in recovery
  • Discovering boundaries and intimacy in recovery
  • Spiritual growth and the11th Step
  • Co-occurring issues such as medication management and/or marriage counseling, physical and medical needs
  • Levels of sobriety and the developmental aspects of recovery
